Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted by two inspectors on December 17, 2019. The on-site inspection was from 10:42 a.m. to 2 p.m. On this date, there were twenty two children present in five classrooms and in the care of five staff members. Two extra staff members were available to assist if needed. The total enrollment for the program was 40 children. The exceptions to compliance were noted on the violation notice and the risks ratings were noted on the supplemental page. There were five staff members' records and five children's records reviewed. Five classrooms were toured and observations were made in two of those rooms. Two staff members and three children were interviewed. The medication and medication paperwork were reviewed for two children. Some items that were reviewed and observed included: required posted information, injury reports, documentation of evacuation, shelter-in-place, and lockdown drills, emergency supplies, first aid kit, warm water temperature, updated emergency preparedness plan and injury prevention plan, resilient surfacing, infant care, free choice activity, books, hand washing procedures, transitions, lunch, and nap.

Based on a review of staff records, the provider did not ensure that a completed Sworn Statement was on file prior to employment and every five years thereafter.
Evidence: 1. The most recent sworn statement on file for Staff #1 was signed and dated 2/5/16, but lacked a response to the four required questions. 2. Administrator confirmed that the sworn statement was incomplete.
Plan of Correction:
The administrator immediately printed a new sworn statement and had Staff #1 fully complete. The oversight was an isolated incident.
