Comments:
An unannounced renewal inspection was conducted on 10/11/2022 from 3:50 pm - 5:17 pm. At the time of entrance there were 20 school age children in care with two staff. A third staff arrived at 4:45 pm. Children were observed playing with board games in the cafeteria. Transition to/from outdoor play was additionally reviewed.
Records were reviewed for three children and seven staff.

Information gathered during the inspection determined non-compliances with applicable standards and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review and interview, the center failed to maintain and keep at the center a separate record for each child enrolled which shall contain all of the required elements.

Evidence:
1. Child 1, who was in care during the inspection, lacked an enrollment record.
a. Administrative staff confirmed that an enrollment record was not available for child 1.
2. The first day of attendance was not documented in the sample size of three children's records reviewed.

Plan of Correction: Administrative staff stated that enrollment dates will be added to children's records. Additionally, an enrollment record will be obtained for child 1.

Standard #: 8VAC20-780-90--A
Description: Based on record review and interview, the center failed to ensure that a written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance.

Evidence:
1. A written parent agreement was not on file for child 1 who was in care during the inspection.
a. Administrative staff confirmed that a parent agreement was not available for child 1.

Plan of Correction: Administrative staff stated that a written parent agreement will be obtained for child 1.
