An unannounced record review inspection was conducted at the facility's central location from 11:07am through 11:44am. Four staff records were reviewed. Violations were identified regarding staff records and tuberculosis screening and these violations appear on the violation notice.
Based on record review the center failed to ensure that each staff has documentation that a tuberculosis screening has been received within the allotted time frame. Evidence: 1 - The record for staff 1 does not contain any documentation that tuberculosis screening was obtained. a. The documented date on the sworn statement or affirmation for staff 1 is August 22, 2019.
