Comments:
An unannounced monitoring inspection was conducted today from 10:15am-11:45am. There were 4 children (12 points) directly supervised by the provider. The home, 1 staff record, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Medications on site were reviewed and found to be in compliance. Children were observed playing freely, being bottle fed, and watching videos. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Based on observation, not all emergency supplies were in working condition.
Evidence: The portable battery-operated weather band radio did not turn on, even after the batteries to the radio were changed.
Plan of Correction:
I will purchase a new weather band radio by the end of the week.
