Comments:
An unannounced monitoring inspection was conducted with the director beginning at 9:45 AM and ending at 11:10 AM. There were thirty-four children in the care of eight staff members. The children's ages ranged from 3 months through ten years old. The infants were observed in exersaucers. The older children were observed playing with toys and watching an age appropriate holiday movie. Five children's records were reviewed. Five staff records were reviewed and in compliance. One medication was reviewed and in compliance. The outside recreation area was not inspected due to weather conditions. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent evacuation drill was documented as 12/19/19. The most recent shelter-in-place drill was documented as 6/6/19. The most recent fire inspection was dated 12/21/18. The most recent health inspection was dated 8/20/19.

Based on a review of five children's records and staff interview, not all of the children's records contained the required informations. Evidence: Child A's record was missing the address and phone number of one parent's place of employment and address for one emergency contact. Child B's record was missing the address for two emergency contacts. Child C's record was missing the policy number for the child's medical insurance.
Plan of Correction:
The center will obtain the missing information from the parents or guardians. The childrens' records will be updated once the information is received.
