Based on record review and discussion with the staff, the facility failed to obtain a follow-up tuberculosis screening/test at least every two years from the date of the first initial screening/test.
Evidence: The most recent tuberculosis screening/test in the record for Staff #2 was dated 08/24/2017; there was not a 2019 follow-up tuberculosis screening/test in the record for Staff #2.
Plan of Correction:
A test has been scheduled for 09/30/2019.
