An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted November 8, 2019 from 6:45 a.m. to 8:30 a.m and November 12, 2019 from 3:00 p.m. to 4:00 p.m. On November 8, 2019 there were 24 children, ages 4 years through 13 years old, were under the care and supervision of 2 staff. Center documents and 7 staff records were reviewed. Based on review of documentation and interview with staff, it was determined that the center did not obtain sworn statements prior to the 1st day of employment and central registry background check results by the 30th day of employment. Evidence: Staff A (start date9/2017), staff B (start date: 9/2016), staff C (start date: 9/2017), staff D (start date: 9/2017), staff E (start date: 9/2018), and staff F (start date: 9/2017) were missing central registry background checks in their records. Staff G's most recent central registry background check was from 2000.
Plan of Correction:
The center will obtain central registry background reports for all current staff as soon as possible.
