Comments:
An unannounced Renewal inspection was conducted on March 4, 2020. The Licensing Inspector arrived in the center at 9:45 AM and departed the program at 12:30 A.M. There were 14 children and 3 staff present at the time of the inspection. The children were observed in their classroom and in the outsider. Staff were observed interacting with the children. Observed activities included: individual-selected center play, hand-washing procedures, snack,and circle time. The inspector reviewed five children's records. Staff records were reviewed at an earlier date and are in compliance. Documentation of fire and health inspections was reviewed.
Based on review of five children's records, the center failed to ensure that all immunizations were signed and dated by the physician or designee.
Evidence: 1. Child # 3 did not have a physician's signature and was not dated.
Plan of Correction:
The FSW will make sure that a signed and dated copy in the child's record.

Description:
Based on review of five children's records, the center failed to ensure that all records were complete as required by the standard.
Evidence: 1. Child # 4 did not allergies and chronic physical problems completed.
Plan of Correction:
The FSW will have the parent to complete the missing information of the enrollment form.
