Comments:
An unannounced monitoring inspection was conducted from 11:30am-1:30pm. There were 53 children and 9 staff. 14 of the school-age children were observed leaving to go on a field trip to I-Hop during the inspection. A sample of 6 children's records, 4 staff records, evacuation drills, emergency materials, the playground, and the physical plant were reviewed. The children were observed making arts and crafts, playing with a dollhouse, playing on the playground, eating lunch (grilled cheese, fresh fruit, salad, and milk), being fed bottles, and taking a nap. Based on a review of staff records, 1 out 4 staff did not have a current tuberculosis screening on file.
Evidence:
1. Staff #4's last TB test was dated 7/24/2017 and there was no documentation of a subsequent TB screening on file.
Plan of Correction:
The staff member transferred from another location and that location was contacted during the inspection. They will look for it and send it to us.
