Comments:
An unannounced monitoring inspection was conducted today approximately 12:30 pm- 2:45 pm. There were 63 children supervised by 7 direct care staff. The physical plant, 3 staff records, 5 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The playground and surfacing was inspected. The children were observed in free play and in center activities. Hand-washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Based on observation, areas and furnishings of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. In the classroom of Toddlers, one blue cube had torn material that was accessible to children.
Plan of Correction:
The mat will be discarded and a new one will be purchased immediately.

Description:
Based on observation, the center did not maintain the diapering area had a nonabsorbent surface for diapering or changing to be used. Evidence: 1. In the classroom of infants had three small tears in the corners of the material.

The diaper mat will be discarded and a new one will be purchased immediately.
