Comments:
An unannounced monitoring inspection was conducted on June 14, 2023 from 1:10pm - 2:00pm. There was two caregivers and five children present. Children were observed freely playing and eating lunch. Two children records and two caregiver record reviewed on this date.
Information gathered during the inspection determined area of non-compliance. Areas of non-complaince is noted on the violation notice.
Based on record review and staff interview, the provider did not ensure child's record obtained all required information.
Evidence: Child #1 and Child #2 were missing home address and telephone number of each parent, parent employer name, address and phone number, name and policy of medical insurance, Written authorizations and emergency contacts addressses and phone numbers.
