Comments:
A mandated Renewal inspection was conducted today from 1:45 pm to 3:45 pm. There were 2 children present (4 points) with one caregiver. Children were observed watching a Peppa Pig video, washing hands, having lunch and napping. Reviewed 2 children's file, 1 caregiver file, 4 pet rabies vaccinations, attendance, emergency evacuation log and supplies, menu, and physical plant. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on record review, the provider did not maintain an up-to-date record for each child enrolled.
Evidence:
Child#2 did not have documentation of an annual review of the child's emergency information. The last update was 7/3/18.

Plan of Correction: I will have the parent update.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the child's record did not contain required information.
Evidence:
Child#1's file was missing the Policy number of the child's medical policy; first day of attendance; and address and phone number for 1 emergency contact.

Plan of Correction: I will ask the parents for the information.

Standard #: 22VAC40-111-90-A
Description: Based on record review and provider interview, the provider did not obtain documentation that the child has been adequately immunized before a child may attend the family day home.
Evidence:
Child#1 did not have documentation that they had been adequately immunized prior to starting care. The provider stated the child started care the first week of September, and the child was present during this inspection.

Plan of Correction: I will obtain the immunization record or the exemption.
