Technical assistance was provided regarding requirements for individual contractors.
Comments:
An unannounced monitoring inspection was conducted today from 9:00am-1:30pm. There were 96 children (infants-5yrs.) supervised by 21 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, circle time, and building. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on review of five children's records, three who are under the age of two do not have documentation of updated immunization record every six months. Evidence: Children #1, 2 and 3 are under the age of two and do not have documentation of updated immunizations every two months, last update was 11/14/17, 5/29,18 and 1/2/19 respectively.

I will get the updated immunization records for all children under the age of two.

Based on observation and review of documentation, 30 minutes of tummy time is not being recorded daily for Infants, who cannot turn themselves over. Evidence: There is no documentation of tummy time for infants who can not turn themselves over.
