Comments:
An unannounced complaint inspection was conducted on 2/2/2024 in response to a complaint that was received by the Virginia Department of Education. Classrooms were observed and interviews were conducted with staff relating to allegations child to staff ratios. Based upon a preponderance of evidence, the information gathered during the investigation does not support the allegation, and the complaint is determined to be not valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days. Complaint related: No
Description: Based on review, the facility did not have documentation of current daily health observation training.
Evidence: reviewed 2 staff records and identified through staff interview that all of the staff took daily health observation, on the same day and that the most recent training expired on 3/20/2023.

Plan of Correction: I have taken steps to ensure that there will always be a staff member on the premise who is up to date with their Health Observation training.

Description: Based on observation, the facility did not ensure that there was at least 12 inches of space between occupied cots, beds, and rest mats.
Evidence: observed that there were 6 cots (groups of two) that were closer than 12 inches during nap time in the Twos room.

Plan of Correction: The 2?s teachers have also been counseled to always ensure that the cots at naptime are 12 inches apart.
