Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 07/23/2019 regarding physical plant. The inspection was initiated on 08/08/2019 and completed on 08/08/2019. On 08/08/2019, the inspector was on site from 9:30am to 10:20am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff records. See the violation notice for more details.

Based on observation, the center did not ensure that one of three staff records had documentation of a negative tuberculosis screening signed by a physician, the physician's designee or an official of the local health department.
Evidence:
The record of staff #3 (start date: 5/1/19) did not have documentation of a tuberculosis screening signed by a physician, physician's designee or an official of the local health department.
Plan of Correction:
I will send [staff #3] to get it done.
