An unannounced monitoring inspection was conducted on July 13, 2022. There were 31 children and 8 staff were present during th einspection. Additional administrative staff was present. The children were observed particpating in multiple activities and outdoor water play . The sample size was 8 staff records.
Based on record review, the provider did not ensure each person have a complete staff health report annually.
Evidence: Staff #8 most recent health report was completed May 10, 2021 which is more the year old.
Plan of Correction:
Spoke with Staff #8 she has an appointment on Monday, July 25th at 8:45 to complete physical.
