Comments:
An unannounced monitoring inspection took place today, October 8, 2019, beginning at 2:40 PM and ending at 5:00 PM. There were 19 children in care today with two staff providing care and supervision. There were three medications onsite during the inspection and those medications were reviewed. All areas that are used for care were inspected, as were five children's records and all staff records, at a separate site, on September 16, 2019. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children were observed during arrival and roll, playing games, and during snack time . There were discussions about children's records and the emergency radio. Description: Based on record review, the center failed to ensure that each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.

Evidence:

1. Documentation of immunization for children's records (child 1 and child 2), were not signed by a physician, his designee, or an official of a local health department.

Description: Based on record review and discussion with staff, the center failed to maintain and keep at the center a separate record for each child enrolled which shall contain all the elements as required by the standards.

Evidence:

1. Children (1, 2, 3, 4 and 5's) records did not contain a home address of each parent who has custody; the form did not have a blank allowing for parents to list addresses, should they differ from the child's.
2. Record for child (2) was missing the name and phone number of the child's physician.
3. Record for child (2) was missing a name, address, and phone number of a second designated person to call in an emergency if a parent cannot be reached. Information for emergency contact number one did not include an address.

Description: Based on observation, the center failed to ensure that the following required non-medical emergency supplies shall be at the center: 1. A working battery-operated flashlight on each floor of each building that is used by children.

Evidence:

1. The battery-operated radio would not function. Staff 1 stated that the "radio must have drained the batteries".
