Technical Assistance:
Please obtain the address and contact information for the father of Child 3 when the father picks up the child or obtain legal custodial documents indicating that the mother has sole custody.

Comments:
An unannounced monitoring inspection was conducted today from 8:40am-10:40am. There were 14 children directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No medications were on site for review today. Children were observed participating in group play, reading books,being bottle fed, and tracing letters. Staff were observed actively engaging with children and welcoming children as they were dropped off by parents.There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review, one of five children's records reviewed was incomplete.
Evidence:
Child 3's record did not contain proof that the child's age and identity was verified.

Plan of Correction: We will ask the parent's to provide the information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all equipment in the center was maintained in a safe and operable condition.
Evidence:
1. A cot in the toddler room was observed to have the mesh cot surface detached from the frame of the cot. The cot will no longer support the weight of a child when in use.
2. The handles on the play kitchen cabinets in the toddler room were secured shut with yarn. The yarn was observed to be loose enough for a child to fit his or her head between the yard and the cabinet door and become entrapped.

Plan of Correction: This will be corrected today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
A spray bottle of bleach and water was observed in an unlocked cabinet in the preschool room.

Plan of Correction: The cabinet was locked during the inspection.
