Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 11/10/2020 and concluded on 11/10/2020. The resource specialist was contacted by telephone to initiate the inspection. There were 47 children present and 9 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on record review, the center did not ensure that a written allergy care plan, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction, was obtained for each child with a diagnosed food allergy.

Evidence:
1. Child #1 has a diagnosed food allergy. The allergy care plan in the record for Child #1 did not contain a physician's signature.
2. Child #2 has a diagnosed food allergy. The record for Child #2 did not contain an allergy care plan.

Plan of Correction: I will give the parents a copy of our allergy care plan form to be completed by the parents and physician. I will also give the parent of the other child a copy of their current allergy care plan so the parents can get the physician's signature.

Description: Based on record review, 2 of 4 children's records did not contain all required parent contact information.

Evidence:
1. The records for Child #1 and Child #2 did not contain the address for each custodial parent's place of employment.

Plan of Correction: I will get the information from the parents today at pick-up.
