Comments:
An unannounced monitoring inspection was conducted on 05/09/2024. There were 25 children, ages 5 years - 11 years, and 3 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during mealtime, and self-directed activities. The inspector discussed the following with the provider: menus and shelter in place. A total of 5 children?s records, and 3 staff records were reviewed. The inspector arrived for the inspection at approximately 3:15 pm and departed at approximately 4:05 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
REPEAT VIOLATION Based on record review and staff interviews, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence: The record for Child 4 documented the first date of attendance as 08/08/2022. There was not documentation of a physical examination in the record for Child 4. Staff 1 confirmed this document was not available for review.
Plan of Correction:
Staff will obtain from home office.
