Comments:
The Licensing Inspector initiated and completed an unannounced monitoring inspection on 3/6/2023. The inspector was on site from 3:30pm until 4:45pm.
There were 41 children under the care of 4 staff. The ages of the children ranged from four to ten years old. The children were observed during outdoor play. Five children?s record and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain repeat background checks after 5 years.
Staff # 1 did not have an updated Sworn statement on file. The last sworn statement was dated 8/15/2017.
Staff #1 did not have an updated Central Registry on file. The last Central registry letter was dated 1/20/2017.
