An unannounced record review inspection was initiated and completed on 2/13/2024. The inspector was on site from 2:30pm until 3:15pm.
The only records reviewed were staff records. Three records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure tuberculosis screening documentation shall be completed within the last 30 days of the date of employment.
Evidence: Staff #1, with a hire date of 2/12/2024 had a tb screening on file with the date of 10/1/2023 during the inspection on 2/13/2024.
