Technical Assistance:
Technical assistance was provided regarding more staff interaction with children and ratio compliance.
Comments:
Today an unannounced renewal study was conducted between 10:00am & 12:00pm. There were 62 children present with 5 staff members.. Children were observed drawing, building, playing on electronics and having lunch. Physical plant, programs, children's records, medications, and emergency drills were also reviewed. Staff records are reviewed annually in the administrative office, however, MAT (medication administration training) and first aid/CPR certificates were available for review. Based on review of eight children's records, one is incomplete. Evidence: Child #1, start date 9/18, does not have documentation of a physical.

Based on observation, staff to child ratios were not in compliance. Evidence: Upon arrival of inspector, classroom was out of ratio, 40 children, 5-8 years old were supervised by two staff members.

Based on observation, food brought from home is not being labeled with child's name. Evidence: Lunch boxes brought from home are not labeled.

We will make sure all food brought from home is labeled.

Description:
Based on observation, tables were not sanitized before each time food is served. Evidence: Tables were not sanitized before children sat down for lunch.
Plan of Correction:
We will make sure to sanitized the table before the children eat lunch or snack.
