Comments:
An unannounced monitoring inspection was conducted today at 11:15 am. There were 3 children directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review, two of three children's records did not contain all required information. Evidence: 1. Child 1's record did not contain the address for one of two listed emergency contacts, the child's health insurance policy number, or documentation that the information has been reviewed by a parent in the past year. 2. Child 2's record did not contain documentation that the information has been reviewed by a parent a in the past year.

I will ask the parents to update and review the information.

Description:
Based on observation, cots, when used for napping did not have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. Evidence: Two cots in use by children were observed with only a top cover.
Plan of Correction:
I will order bottom covers on Amazon.
