Comments:
An unannounced non-mandated inspection was conducted on 02/26/2024. There were 7 children, ages 11 months- 4 years, totaling 18 points and two caregivers present during the inspection. Two inspectors arrived for the inspection at approximately 11:15 am and departed at approximately 12:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review, the provider failed to obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements: once every six months for children under the age of two.
Evidence: 1. The most recent immunization record for Child 2, 21 months, was dated 05/30/2023. There was no additional immunization record available for review. 2. The most recent immunization record for Child 3, 23 months, was dated 06/28/2023. There was no additional immunization record available for review.
Plan of Correction:
Provider will obtain current immunization documentation from parents.
