Technical assistance was provided regarding proof of identification documentation.
Comments:
An unannounced monitoring inspection was conducted today from 10:55am-1:15pm. There were 61 children directly supervised by 12 staff and all classrooms were in ratio. The physical plant, outdoor playground areas, programming, menus, 4 staff records, 7 children?s records, 5 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed engaging in age appropriate center play and listening to story time. Infant care including feeding and diapering procedures was observed. The school age children were off site on a field trip. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on review of 6 children's records, 1 child's record was incomplete.
Evidence:
1. Child #1's record was missing a complete address for 1 of 2 required emergency contacts.
2. Child #1's record did not contain documentation that proof of identification was verified within 7 days of child's enrollment. Child's first day of attendance at the center was 1/7/2019.
Plan of Correction:
We will verify the birth certificate and get the emergency contact address by the end of today.
