Comments:
A complaint investigation was completed on January 28, 2020 from 1:00pm-3:20pm in conjunction with another local governmental agency. The complaint consisted of allegations received by the Northern Licensing Office on January 24, 2020 regarding supervision and the care of children. Based on staff interviews and review of documentation, a preponderance of evidence supports the allegations regarding the care of children. Therefore, the complaint is determined to be valid. Additional violations were cited not part of the complaint investigation. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-340-A
Complaint related: Yes
Description: Based on staff interviews and review of screenshots of the center's video recording system, staff have not ensured infants' care, protection, and guidance.

Evidence:
1. On Thursday, 1/23/2020, Staff #1 in the Infant B room did not ensure the care, protection, and guidance of the infants in the room when infants were able to push an unoccupied crib out of the corner and move it into the path of other infants. Between 1 to 2 infants at a time were able to move the unoccupied crib in multiple different positions within an area of approximately 5 feet. No staff were observed in the screenshots as the infants were moving the cribs around the room.
2. Staff #1 stated "I don't know why they do that, they still manage to move the crib, they will team up to move it." Staff #1 further stated that infants moving cribs is an everyday occurence.

Plan of Correction: Crib locks were placed on wheels immediately and was addressed next business day. Staff was also taken from the room and re-evaluated for their positions. Training provided.

Complaint related: No
Description: Based on staff interview, staff have not ensured that safety straps are always securely fastened when children are seated at a feeding table.

Evidence: Staff #2 stated that she doesn't always hook the safety straps on the feeding table. In addition, Staff #2 stated she did not hook Child #1, a child under 12 months, into the feeding table on 1/23/2020 at lunch time.

Plan of Correction: Straps were supplied to the seats in the infant room with routine checks to ensure that all children in seats are safe and secure.

Complaint related: No
Description: Based on record review, 1 of 2 staff records reviewed did not contain documentation that an out of state central registry search request was sent for all staff who have resided outside the state of Virginia within the last 5 years.

Evidence: Staff #1 (Date of Hire: 9/4/18) has resided outside the state of Virginia within the last 5 years and there was no documentation an out of state central registry search request was sent.

Plan of Correction: CPS registry check was sent off out of state and copy placed in file.
