Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on August 13, 2021 and concluded on September 13, 2021. A self-reported incident was received by the department regarding allegations in the areas of Administration, Special Care Provisions / Emergencies and Supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and a violation was issued for Supervision. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.
Based on interviews and documentation review, the licensee did not ensure all children under 10 years of age shall always be within actual sight and sound supervision of staff.
Evidence: Staff #1 confirmed during a fire evacuation drill on July 19, 2021, Child #1 was left in the classroom without supervision of center staff. Center staff evacuated 8 children from the Toddler room during a fire drill. Once staff and children were outside, Staff #2 reviewed center documentation (digital child roll list), conducted a head count and realized Child # 1 was missing. Staff #3 went inside to the classroom and retrieved Child #1 who was left sleeping in a crib. Child #1 was left unsupervised for approximately one minute.
Plan of Correction:
On August 16th we held a training on our emergency preparedness policy. Staff member #2 was released. Staff involved received written reprimands on August 13. All rooms received new color cards.
