Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated on October 4, 2021 and concluded on October 12, 2021. There were 63 children present with16 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and staff interviews, the licensee did not ensure staff shall be evaluated by a health professional and be issued a statement the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence: The following staff did not have a TB screening conducted prior to employment. 1) The TB screening for staff #2 was dated 10-1-2021 and date of employment was 9-27-2021. 2) The TB screening for staff #3 was dated 10-1-2021 and date of employment was 9-21-2021. Documentation was confirmed by center director.
Plan of Correction:
Staff 2 and 3 have a current TB. We now understand that the TB leniency no longer stands.
