Comments:
An unannounced complaint inspection was initiated in the center from 10:16 AM until 12:45 PM on November 20, 2023 and again on December 19, 2023 from 2:05 PM until 4:20 PM and completed on January 11, 2024 regarding concerns of Administration, Special Care Provisions/Emergencies, Special Services and Programs. During the inspection the following were completed: interviews with administrative staff, center staff, a review of records/documentation, observation of the infant classrooms. The preponderance of the evidence gathered during the investigation does not support the allegation related to Administration, Special Care Provisions/Emergencies, Special Services and Programs and therefore, the complaint is determined to be NOT valid.

The inspection determined non-compliance with applicable standards or law, not complaint related were cited in the area of Special services. The violations were documented on the violation notice issued to the program.

Description: Based on observation and staff interviews, the licensee did not ensure the record of each child on formula shall contain the brand of formula and the child's feeding schedule.

Evidence: During the inspection on 11-20-2023, 8 infants on formula were observed and their record did not include the brand of formula they were using. This was confirmed by the Center Director.

Plan of Correction: Infant feeding forms provided by CACFP are now on file and printed.

Description: Based on observation and staff interviews, the licensee did not ensure prepared infant formula shall be refrigerated, dated and labeled with the child's name..

Evidence: During the inspection on 11-20-2023, 5 bottles with formula were observed 3 did not include the date.

Plan of Correction: Teachers reminded to label / date bottle's @drop off.
