Comments:
On March 4, 2020 an investigation was initiated in response to a self reported incident involving care of Children and operational responsibilities. There was not sufficient evidence to validate the complaint.However, violations found during investigation are indicated in the violation report. Add usual verbiage regarding your contact info. Based on review and interview with provider it was determined that all required information for child record was not maintained.
Evidence: Provider stated that they only had a physical and the cell phone number of 1 parent as this child was placed with her on the emergency bases.
During inspection the following information was unavailable: address, parents' workplace and phone number, 2 emergency contacts, proof of identity, name and policy number for medical insurance, and parent's signed acknowledgment of receipt of policies and procedures and written authorization for emergency medical care.
Plan of Correction:
I will not accept children like this again. I usually make sure the information is correct.
