Comments:
On December 15, 2020 a virtual monitoring inspection was conducted. Due to the COVID 19 pandemic the inspector was not on site; the inspection was conducted via mobile video application as well as email and phone correspondence. During the inspection there were 4 children present directly supervised by the provider and assistant. Child, assistant, provider and household member records were reviewed. The home as well as emergency procedures and supplies was also inspected. Violations were indicated in the violation notice.

Based on review of assistants records it was determined that required certifications were not maintained.
Evidence: During inspection the assistant's observed caring for children cardiopulmonary resuscitation (CPR) certification was expired as of September of 2019.
Plan of Correction:
Both the instructor and the assistant were out of the country I will make sure they get the training as soon as possible.

Based on observation, cleaning materials were not inaccessible to children.
Evidence: During inspection disinfectant wipes were observed on the top of toilet in a bathroom accessible to the children in care.
Plan of Correction:
During inspection provider moved the container to a higher shelf.
