Comments:
An unannounced monitoring inspection was conducted on Monday, November 4, 2019 from 2:30 PM to 3:57 PM to determine compliance with previously cited violations. Upon arrival, children were observed arriving to the center from local elementary schools. Observations of the procedures for children exiting the buses and transitioning into the center were conducted. Five staff emergency contacts and health information were reviewed. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In classroom #1, four bottles of Isopropyl Rubbing Alcohol labeled "Keep out of reach of children" and "Flammable" and one bottle of Sanitize Germicidal Solution labeled "Hazard to humans and domestic animals,? ?Caution? and "Keep out of reach of children" were unlocked on a top shelf accessible to the children.
2. In classroom #2, one can Lysol Disinfectant Spray labeled ?Flammable," "Keep out of reach of children," "Hazard to humans and domestic animals? was unlocked on a bottom shelf accessible to children.
3. In classroom #2, one can Rust-oleum spray can labeled ?Danger,? ?Extremely Flammable,? ?Vapor Harmful? and "Keep out of reach of children" were unlocked on a top shelf accessible to children.

Description: Based on observation, the center did not ensure that cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children.

Evidence:
In classroom #1, three boxes of antacid pain relief aspirin labeled "Keep out of reach of children," were on a bottom shelf accessible to the children.
