Comments:
A monitoring inspection was conducted on 10/30/23 from 10:45 am to 12:00 pm. There were 12 children present, with 4 staff supervising. The children were observed engaged in movement activities, arts and learning. The infant classroom was open today. There were no children with allergies or medications. A total of 2 children's records, 3 staff records, attendance and emergency drill records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain documentation of a physical exam for each child within 30 days of enrollment. Evidence: 1. Child # 2's first day of enrollment was 7/10/23. 2. Child # 2's record did not have documentation of a physical exam.

The child does not attend everyday. We will get the physical exam copy from the parents.
