Comments:
An unannounced monitoring inspection was conducted today from 2:30pm-3:45pm. There were 11 children (39 points) supervised by 3 direct care staff. The home, 2 staff records, 7 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping. Bottle feeding procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Based on observation, the menu for the current one-week period did not contain documentation of all required information.
Evidence: 1. The menu was not dated. 2. The menu did not list substituted food. On the date of inspection, the children were provided meatballs for lunch and the menu listed chicken nuggets.
Plan of Correction:
Parents are verbally told every day what items will be served. We will be sure to date the menu and write any changes.

Evidence: Child #8's (age: 4 months old) record did not contain documentation of medical insurance information and parental acknowledgment of the receipt of the information required by 22 VAC 40-111-70.

Forms were given to parents and were not returned. I called parents today and forms will be returned tomorrow.

Description:
Based on observation, occupied cribs were not at least 12 inches from each other.
Evidence: Three cribs placed in the corners of the naptime room were touching adjacent cribs, and did not have any space between one another.
Plan of Correction:
Moving forward, will be sure to move the cribs.
