Technical Assistance:
Discussed the full address should be written on the Provisions of the emergency plan form for parents. Discussed CPR/FA in person training is required. Discussed inspections and allowing agents in to inspect at all reasonable times.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced monitoring inspection was conducted today from 10:12am-12:07pm with the provider on site. There were 9 children (32 points) present with 2 caregivers from approximately 10:12am to 10:50am and 3 caregivers from approximately 10:50am to 12:07pm. A sample of 4 children's files and 3 caregiver/household files were reviewed. The children were engaged in the following: laying in cribs, drinking bottles, being fed snack and playing with toys. The physical space, attendance records, drill record, menu and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-800-750-A
Description: Based on observation, the provider did not meet all requirements when using sunscreen for children in care.
Evidence:
1. Two Children had sunscreen labeled with their names, but did not have written parent authorization.

Plan of Correction: Parents provided the over the counter sunscreen. Parents provided the authorization forms as well. Please elaborate on this volition [sic] or remove.

Description: Based on review of the menu, the family day home did not follow the most recent age appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:

Plan of Correction: It is not clear what this violation is. Either provide the appropriate standard number or remove violation.

Description: Repeat Violation
Based on observation, information contained in a child's record was not kept privileged and
confidential.
Evidence:
1. The supplemental page from the inspection dated 1/31/2024 was posted on the wall. This
is considered confidential.

Plan of Correction: Noted, we will remove the supplemental page.

Description: Based on record review, one child's record did not contain all required information.
Evidence:
1. Child #2's file (start date 3/18/2024) was missing the medication insurance policy number.

Plan of Correction: Insurance policy number has been included in the documents.

Standard #: 8VAC20-800-70-B
Description: Based on interview, the provider did not obtain the parent's written acknowledgement of the receipt of the Information for Parents form.
Evidence:
1. Child #1's file (start 6/17/2024) did not have the parent's written acknowledgement of the receipt of the Information for Parents form.

Plan of Correction: Noted, we will provide the information required.

Standard #: 8VAC20-800-80-A
Description: Based on record review, the provider did not obtain from the parent the names and location of previous day care programs and schools the child attended within seven business days of the child's first day of attendance at the family day home.
Evidence:
1. Child #1's file (start date 6/17/2024) did not have the names and location of previous day care programs and schools the child attended.

Plan of Correction: The baby did not attend any previous day care programs or schools.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child attended the family day home.
Evidence:
1. Child #3 (start date 9/06/2023) had immunization records dated 9/12/2023 and not before the child attended.

Plan of Correction: Noted.

Standard #: 8VAC20-800-480-D
Description: Based on observation and interview, equipment used by children were not assembled, maintained, and used in accordance with the manufacturer's instructions.
Evidence:
1. One crib base in the kitchen area was taped with duct tape across the entire width of the crib in two places covering two large cracks of the base under the mattress.
2. The four cribs in the bedroom were missing the base to hold the mattresses. The provider had installed board slabs to replace the base and were not maintained in accordance with the manufacturer.

Plan of Correction: We will look to see if the base board is sold by the manufacture and buy if available.

Standard #: 8VAC20-800-520-C
Description: Repeat Violation
Based on observation, 7 out of 11 cribs did not meet the crib requirements.
Evidence:
1. 7 cribs did not have the manufacture date or documentation to document the cribs meet the
US Consumer Safety Commission standards at the time they were manufactured.

Plan of Correction: We will try to contact manufacture to see if they can provide the documentation.

Standard #: 8VAC20-800-530-D
Description: Based on observation and interview, linens were not cleaned and washed when soiled.
Evidence:
1. Two cribs were soiled and stained and one crib had a cheerio in the crib. The provider stated they wash sheets three times a week.

Plan of Correction: Noted. We will comply with the regulations which indicate weekly wash and when dirty. We have removed the cheerio and will replace the sheets as we are unable to remove the strawberry stain.
