Based on record review, the center did not ensure three of six staff records had all of the required documentation. Evidence: 1. The record of staff #2 (start date: 01/30/2019) did not have written information to demonstrate the staff possesses the orientation training required by the job position. 2. The record of staff #3 (start date: 08/06/2019) did not have written information to demonstrate the staff possesses the orientation training required by the job position. 3. The record of staff #5 (start date: 05/30/2018) did not have written information to demonstrate the staff possesses the orientation and programmatic training required by the job position. 4. The record of staff #6 (start date: 01/30/2019) did not have written information to demonstrate the staff possesses the orientation training required by the job position.

Evidence: The record of staff #6 (start date 1/30/19) did not have documentation of an out of state child abuse and neglect search request. The staff's record indicated that the staff lived out of the state of Virginia within five years prior to the date of hire.
