Comments: This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, and documents review. A monitoring inspection was made on 9/2/2021. The director was contacted by telephone and a virtual inspection was conducted. There were no children present as this site is not operating at this time due to low enrollment. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
