An unannounced monitoring inspection was conducted today from 9:00am-11:00am. There were 30 children supervised by 7 direct care staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, table top activities and coloring. . Areas of non-compliance are identified in the Violation Notice. Based on review of six children's records, one does not have documentation of additional immunizations once every six months for a child under the age of two years. Evidence: Child #1(DOB 4/18/18), does not have documentation of updated immunizations every six months, last update was 10/22/18.
Plan of Correction:
We have contacted the parent 12/16. 12/27 and 12/30 to get the update. Parents said the child is up to date, they will get a copy to us.
