Comments:
An unannounced monitoring inspection was conducted to investigate complaint allegations regarding children's records, supervision, safety and operability of equipment within the center. The investigation was conducted with the director beginning at 8:25 AM and 9:35 AM. Two staff interviews were conducted. Five children's records were reviewed. Two staff records were reviewed and in compliance. Observations of the classroom and physical plant were conducted. Two additional staff interviews were conducted on July 17, 2019 beginning at 1:50 PM and ending at 2:30 PM. The allegations regarding children's records were valid. The allegations regarding supervision and the condition of the equipment within the center were determined to be invalid.

Based on a review of five children's records, the center did not have complete records for all children enrolled. Evidence: Child A and Child B's records were missing documentation of viewing the child's proof of identity and age.

Documentation was received and placed in the children?s? files on 07/17/19.
