Comments:
A monitoring inspection was initiated on 07/24/2023 and concluded on 07/24/2023. There were 18 children present, ranging in ages from 5 years to 10 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on record review, the center failed to ensure that each report shall include the dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
Evidence: 1. The record of Child 4 had immunizations that were not signed by a physician, his designee, or an official of a local health department. 2. Staff 4 confirmed there was no physicians signature.
Plan of Correction:
Upon registration, the immunizations and physical's will be reviewed to make sure there is a signature from a doctor on it.
