Comments:
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 1/29/2024 relating to an injury, behavioral guidance, forbidden actions and transporting children off premises without permission. Interviews were conducted with the provider and children and documents were reviewed.
The information gathered during this investigation regarding behavioral guidance and forbidden actions does not support the allegations, so the complaint is determined to be not valid.
The information gathered during this investigation regarding transporting children off premises without permission and the injury determined non-compliance with applicable standards or law and two violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on interview, the provider failed to record in the child's record an injury or accident sustained by a child while at the family day home that requires first aid. Evidence: 1. On 1/27/2024, a child was injured at the provider's home and the provider did not record the injury and place in the child's record. 2. The provider stated a child was injured on 1/27/2024 while in her care, but did not fill out an injury report with the required information. She stated she told the parent verbally when they picked up the child.
Plan of Correction:
Going forward, the provider will record any and all injuries in the injured child's record.

Description:
Based on interview, general written permission was not obtained from the parent of each child for the provider to take the child off the premises of the family day home that included a general written permission form that lists regularly scheduled trips and the driver, if the child is to be transported. Evidence: 1. The provider admitted taking two children off the premises of the family day home without written permission. (Target and Costco and a friends home).. 2. The parent of the two children confirmed the provider did not have written permission to take the children off the premises of the family day home. They had permission to go to the bus stop only.
Plan of Correction:
The provider will not take children to any store or anywhere to run errands without written permission from the parent(s).
