An unannounced complaint investigation was conducted in response to a complaint received on 5/21/19. Observations were made, reports reviewed and interviews were conducted in relation to the allegations of cleanliness of the center, areas of the center being maintained in a safe, operable condition and injuries of children sustained while in care. The preponderance of evidence gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid".
