Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 08/20/2020 and concluded on 08/21/2020. The director was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Based on record review, 2 of 2 children's records did not include the address for each custodial parent's place of employment.
Evidence: The records for Child #1 and Child #2 did not contain documentation of the mother and father's work addresses.
Plan of Correction:
I will give the parents the form and ask them for the information.
