Comments:
An unannounced monitoring inspection was conducted on May 6, 2024 from approximately 10:30 am to 11:45 am. There were 15 children in attendance and a total of 2 staff present. 2 staff records and 5 children?s records were viewed. All areas of the center were observed including classroom, bathrooms, and the playground. The children were observed playing "musical chairs", learning about animals, having morning meeting, and playing in centers, while the licensing inspector was on-site.

The first aid kit was inspected and found complete. Evacuation drills were conducted monthly with the most recent conducted on 04/30/24.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 05/13/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The bathroom used by children had a piece of molding approximately 1 foot in length broken and laying on the floor.

Plan of Correction: Management will have maintenance go to the center to repair molding.
