Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was conducted on 3/11/2021. The Assistant Director was contacted by telephone to initiate the inspection. There were 25 children present and 4 staff. The inspector emailed the Assistant Director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4pm on Thursday 3/18/2021. Please email me at morgan.bryson1@dss.virginia.gov with any questions.
Based on record review and staff interview, the center did not obtain the results of an out of state sex offender registry name search for each staff member who has resided outside the state of Virginia within the last 5 years.
Evidence: Staff #2 (Date of Hire: 6/8/2020) has resided in West Virginia within the last 5 years and did not have a sex offender registry name search on file.
Plan of Correction:
Ran staff's sex offender registry and added it to file.
