Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 12/02/20 and concluded on 12/07/20. The director was contacted by telephone to initiate the inspection. There were 54 children and 12 staff present.
The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children and 2 staff records to ensure documentation was complete. The information gathered during the inspection determined one violation with applicable standards or law.
On 11/25/20 the center self reported an incident involving an allegation of a child sustaining a bruise and red mark from a staff. The investigation was conducted remotely and along with the local department of social services. Interviews and a review of documents were conducted and there were no violations associated with this situation.
Based on a review of two children's files, it was determined that one child did not have a physical on file within 30 days of enrollment. Evidence: Child A enrolled on 8/17/20 and as of 12/7/20 there was no physical on file.
Plan of Correction:
