An investigation was initiated from 12:15 pm - 2:30 pm in response to a complaint received involving a forbidden action. Interviews were conducted, video footage, documentation, staff and child records were reviewed. The complaint regarding a forbidden action is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interviews and review of video footage it was determined that staff engaged in an action that is forbidden. Evidence: On Friday, September 1, 2023 between 12:11 pm - 12:15 pm Staff A is observed on video footage roughly handling children in the Infant B classroom. 1.) At 12:11 pm Staff A is seen roughly picking up child A (Age 14 months) and roughly places them on their cot. 2.) At 12:15 pm Staff is seen placing child B (Age 14 months) on their cot. When child B attempts to get up, staff A roughly pushes the child's head down on their cot.
Plan of Correction:
-Teacher is on suspension -Teacher will complete assigned training -Teacher will have assistance during nap time
