Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 04/05/2021 and concluded on 04/06/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. If you have any questions regarding this inspection, please contact the Licensing Inspector, Maria Robles at maria.robles-lopez@dss.virginia.gov. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, children's records did not contain all required information. Evidence: 1) The record for Child #2 did not contain the name for one custodial parent's place of work and the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.
Plan of Correction:
Will contact the parents to obtain the information.
