Comments:
An unannounced monitoring inspection was conducted on 04/10/2024. There were 20 children, ages 6 months-5 years, and 5 staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, physical plant, emergencies and nutrition. A total of 5 children?s records, 5 staff member records, and 1 pet record were reviewed. The children were observed during breakfast, in free choice play, and participating in story time. The inspector arrived for the inspection at 8:00 am and departed the center at 10:25 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and that documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence: The record for Staff 1 contained a documented hire date of 03/14/2024. The tuberculosis screening was dated 03/20/2024; the screening was not completed at the time of employment.

Plan of Correction: Moving forward will get the TB screening at the time of hire.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child 3 contained documentation of a diagnosed food allergy. The record did not contain the required written care plan by a physician.

Plan of Correction: Today is the child's last day. Moving forward allergy care plans will be required before the child can attend.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the center failed to ensure that children under the age of 10 years shall always be within actual sight and sound supervision of staff.

Evidence: Child 5 (3 years) was observed receiving services from an outside agency in an upstairs classroom. During the time that Child 5 was receiving the services in the upstairs classroom they were not within actual sight and sound supervision of staff.

Plan of Correction: Will have parents give authorization for the service agency to sign children in and out for services.
